HSHSL Incident Report
Submit an Incident Report form to describe any injury, property damage, security occurrence, or unruly behavior at the HSHSL. The report, which contains a detailed and factual description of the incident, is submitted AFTER the incident has occurred.
- If you need immediate assistance, call 911 for emergencies.
- Contact the UMB's Title IX Office to report a concern regarding misconduct, discrimination/harassment, and more.
- Always complete an incident report when UMB Campus Police, Baltimore City Police, or 911 is called.
When Shall I Complete an Incident Report?
Complete an incident report when:
- You witness an incident
- You have been involved in an incident
- A library user or staff member reports an incident to you
Activities That Warrant Submitting a Report
These are examples of activities that warrant submitting an incident report:
- A library user or group of users become unruly, disruptive, or threatening
- A library user does not follow library policies
- A non-HSHSL staff is found in a restricted area of the library
- A person is injured within the HSHSL or outside on the nearby plaza or lawn
